Parole Packet and Planning Document Downloads

This page includes downloads to help you complete a parole packet for your incarcerated loved one, and guides to help you organize information related to your loved one’s incarceration. The Parole Packet Checklist contains suggestions of documents that can be added to a parole packet for your loved one. Every parole packet should be tailored to your specific situation and this checklist should only serve as guidance.

The Relevant Courses Document helps document and organize classes, personal development, and certifications recieved while incarcerated, as well as, additional courses/skills.

The Home Plan Worksheet allows the residents of the address the inmate plans to parole to, to attest to following the rules and regulations of parole. Home plans should include pictures of the home, any vehicles provided and family living at the address.

The Parole Denial Document allows the inmate to show the Parole Board that they have a “Plan of Action” if they are denied parole. This helps to show the Board accountability, the inmates organizational skills and self-motivating factors to receiving a positive parole during their next review.


Planning Documents

The Information Tracker helps you keep all your loved one’s information in one place. You can utilize the information when communicating with the unit, advocacy groups and outside organizations.


Reintegration Documents

The Employment Plan Worksheet can help outline the inmate’s plans for seeking employment upon release. It includes steps for job searching, industries or positions of interest, any contacts or networks they plan to utilize, and strategies for addressing potential employment barriers (e.g., discussing their incarceration with potential employers).

The Education and Skills Development Plan allows for the inmate to detail their plans for further education or skills training. This could include identifying specific courses, institutions, vocational training programs, or online learning resources they intend to use to enhance their employability and personal development.

The Support Network Worksheet and Community Engagement Plan contains a list of individuals and organizations that will support them upon release, such as family members, friends, mentors, or support groups. It also details plans for volunteering, joining community organizations or clubs, or participating in community events.

The Goals and Milestones Worksheet helps the inmate articulate their goals related to employment, education, personal development, and other areas, along with steps to achieve these goals and milestones to measure progress.

The Relapse Prevention Plan is for inmates with a history of substance abuse, a detailed plan for avoiding relapse is crucial. This worksheet can include triggers to avoid, coping strategies, and information on attending support groups like Alcoholics Anonymous or Narcotics Anonymous. The Healthcare Plan Worksheet will outline how the inmate plans to address any ongoing health needs, including scheduling appointments with healthcare providers, obtaining necessary medications, and enrolling in health insurance if needed. This document does not constitute medical advice.(Developed w/ direction and input from an APRN and Medical Insurance Specialist)

The Legal Guidance Plan can help organize information about parole meetings, court dates, restitution payments, or other legal requirements. This document does not constitute legal advice. (Developed w/ direction and input from a Family Attorney and Paralegal)

The Financial Planning and Budgeting Worksheet is crucial for successful reintegration and to increase financial literacy. This worksheet can help the inmate plan a budget based on expected income and expenses, outline strategies for saving money, and detail any steps they will take to manage or avoid debt.
